Sports Medicine
Pearland ISD athletic trainers take care of injuries for all athletes grades 7-12. Athletes are required to report all injuries to the athletic trainers for proper care. Each athletic training room has a set time for athletes to be seen, and the coaches are notified of all injured athletes and times they came in for treatment or rehabilitation. The Pearland ISD athletic trainers are all licensed by the Texas Department of State Health Services and are dedicated to the health and wellbeing of all district athletes.
Dawson High School Sports Medicine
DHS athletic training consists of a considerable responsibility that requires hard work, self-discipline and a commitment to the program. As athletic trainers, it is our duty to give the highest level of athletic and sports medicine care to the high school athlete. This includes the prevention, care and rehabilitation of athletic injuries.
Online Forms
UIL/PISD paperwork for the 2024-2025 school year must be completed online for each athlete before they will be allowed to participate in any off-season, in-season, or try-outs.

The parent/guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.
Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.
Physical Exam & Medical History
The physical exam and medical history must be completed on the UIL/PISD approved forms and turned into the appropriate personnel before the athlete will be allowed to participate in any off-season, in-season, or try-outs. Physical exams must be performed by either a Physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, or a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners. Examination forms signed by any other health care practitioner, will not be accepted. Physical exams must be dated on or after April 1st, 2024.

You will not be allowed to participate in any off-season, in-season, or try-outs until both the electronic form and the physical exam/medical history form is turned in.

Pearland High School Sports Medicine
The PHS Sports Medicine Team is a very important part of the success of the Oiler athletics program. We are responsible for the prevention, care and rehabilitation of all athletic injuries that our athletes encounter. Student athletic trainers assist the licensed athletic trainers in taping, administering treatments, providing first aid, evaluating injuries, stretching and computer record keeping.
Required Paperwork
Before athletes participate in any in-season or out-of-season workout, they must have the physical form with eligibility rules and the insurance fee on file with the athletic trainers. Athletic physical forms can be picked up at the administration building and all junior high or high schools.
Athletic Fee
The athletic fee is $40 for an individual athlete and $60 for families with more than one athlete. Fees will be paid through your Skyward account and can be paid with other school fees.
